The time traveller has been stranded for two years now, Saki's mother is ill and Minakata requests to examine her. The patient is uninterested in her own wellness. Others are resolve to improve her nutrition and will bake for her. The magic elixir is junk food from the future. Saki herself is homeless and ostracized as she remains the subject of rumours and gossip. Her case is not helped by her own family. The land's political turmoil continues.

The doctor is invited to present his cure for a disease to none other than the princess of the realm. He hesitates, but is persuaded noting the need for money and also Nokoze's lack of a proper home. He does need a proper and formal kimono for such an occasion. The doctors are spending their day baking doughnuts. Will the princess accept the treatment and will the women all be whole after the events?

The doctor attempts to fight cholera and eradicate it from Edo and Japan, but falls prey to the deadly disease himself. It is only his own instructions and demonstration of modern medicine that allows Saki to come to the rescue. With his reputation cemented he is asked to come forth and do things on a more scalable manner for the advancement of medicine. However, tragedy and confession are woven into the story.

With his reputation cemented the doctor is taken to a brothel in order to examine a prostitute who is feeling ill. There the doctor meets a woman who closely resembles his girlfriend from his time. The proprietor has also fallen ill and the group debate whether to operate on him or not. It is not easy explaining the procedure a hundred years before it is invented.
